I ran it through a bunch of filters and effects in Premiere Pro, but unfortunately the original video is so dark in those areas, that any brightening creates a bunch of artifact. I was able to brighten it enough to see what appears to be either a small piece of toilet paper on her labia, or possibly some cream.

That's really tough. The video is really dark and grainy, just on the cusp of seeing any clear detail. That said, with a combination of contrast adjustment, denoise, and upscaling, this is the best I've been able to manage from the two best shots in the video:
