She's real, Onlyfans doesn't accept face swapping with AI. Taking someone else's image and swapping faces isn't allowed on Onlyfans because you need to prove it with ID to receive the profits!
Only fans only accept AIs if the "character" is 100% AI-created, without using a model or stealing images from other girls, but then it's marked on the page that the creator is an AI.

If you see any profile that uses someone else's photos with AI-generated face swapping, you can report it by sending the original image to Support.
