The two worst I've seen looking back are Ava Devine and Holly Body. It's really a shame since both of them were absolutely beautiful and likely would have aged to become even hotter but then got work done. Ava I could understand wanted to get a reduction and could have left it at that and kept her face exactly the same, she was gorgeous. Then she got work done on her face and tried to go big again which mutilated her breasts and nipples. Holly Body unfortunately just kept getting work done throughout her career when she genuinely didn't need it.
